Spa Medical Questionnaire

To make your arrival at the Spa even easier we request that you fill out your medical form online 48  hours before your Spa treatment/day. If you booked via our Contact Centre and disclosed a medical condition/allergy they will have advised you on whether a Doctor’s note may be requested to clear any person with a known condition.

Please fill in the form and bring along any relevant paperwork such as a Doctor's note on the day if necessary. 

All guests are required to complete a medical questionnaire, before using any facilities or undergoing any treatments - failure to fill in the online form 48hours prior to your visit may result in a shortened treatment time.
